Intake
Add an intake
The fields on the intake form and what each captures.

Select New intake to open the form.
Participant details
- First name and last name (required), and email (required, a valid address).
- Phone (optional, 8 to 15 digits), date of birth, gender, and NDIS number (optional, 9 digits) if known.
What they need
- Services requested: tick any that apply (Community Access, Short Term Accommodation, Group Activities, Support Coordination, Specialist Support Coordination).
Source and notes
- How did you hear about us?: where the enquiry came from.
- Internal notes: anything useful for the team.
When you open an existing enquiry to edit it, you can also set its stage, priority and next contact date.
Select Create intake. The enquiry starts at the New stage.
